| Verification of hair collection for DNA testing form - required for all horses undergoing DNA testing for the purposes of registration with UWPBDA Ltd. | |
| DNA Kit - to be sent with hair samples
and payment to the US lab.
Import Permit - to be included with hair samples (you will also need to request an Australian customs permit from the post office when you post the sample) |

Foal recording application - required for all colts AND all horses / ponies under 2 years of age with GREY, ROAN, APPALOOSA, PALOUSE, UNKNOWN or UNPROVEN parentage must be foal recorded. All others may be permanently registered from birth |

Permanent registration - entires 2 years of age and over (all entires under 2 years of age are required to be foal recorded up until 2 years of age)
